Practice nowThe Form Filling exercise

Who it is used with

Forms are everywhere: the doctor, the pharmacy, the library. Form practice is used with any type of aphasia, at whatever level suits.

Start on Level 1 with the Sign-In Sheet: two fields, tap the answers.

How to use it

Pick a form

From a two-field sign-in sheet to an eight-field library card. Then tap Start Practice.

Fill each field

Copy from the reference card next to the form: tap, finish, or type the answer, depending on the level.

Tap Check Form

Each field shows right or the right answer. Try again, or go on to the next form.

Modes

Pick whose details to use above the forms.

Scenario

Fill in forms for a made-up person, with their details on the reference card.

My Info

Fill in forms with your own details. Someone types them in once, at the start.

Ways to get through it

There is no time limit, and the answers are always on the reference card.

Show

On Level 2, the Show button next to a field fills in the answer.

Levels

In Settings, tick SLP mode: Level 1 Word Bank (tap the answer), Level 2 Guided (the start is filled in, type the rest), Level 3 Independent (type it all). The last level you used is remembered.

Print

Print blank forms to practise on paper.

The method behind it
