Think of sounds as the bricks words are built from. Speech therapists work at this level constantly: what sound does it start with? Does it rhyme with hat? Phonological work is a standard, well-studied part of adult aphasia treatment, because strengthening the sound level can support word-finding above it. It's also why "starts with c…" is such a powerful cue — if the sound system is strong, a first sound can unlock the whole word.
A word's first sound is a doorway to the whole word. When the sound system below your vocabulary is strong, a small phonological cue can trigger full retrieval — which is why cueing hierarchies in speech therapy lean on it.
